Shared Hosting Service Disruption

1026hrs: One of our shared servers is offline and is failing to respond. An engineer is currently on his way to the data centre suite to investigate. More details to follow.

1140hrs: The server is now back online and we are investigating the cause of the outage.

 

Intermittent Disruption on Shared Server 

A number of customers are experiencing intermittent disruption of services due to instability of one of our shared service servers. We are currently investigating the cause of this and an engineer is on his way to the data centre. We apologise for any inconvenience that this is causing and will update this page once we have more information.

Update 12/11/07:

After carrying our some remedial work the server is now stable. We will continue to closely monitor the status.

 

Service Outage

A service outage lasting approx 120 minutes occured at 1630hrs. This is now resoved and an analysis of the root cause will be posted shortly.

Update 20/9/07: 

The root cause was a failure on a third party carrier providing one of the interconnecting networks which affected all network traffic, we have now terminated our contract and service with this supplier as a result of this issue (resilience is maintained across our remaining carriers).

One of the actions from the incident review is to implement a separate management network to ensure access to externally hosted services, removing any dependency on networks linking the data centres. This service improvement activity will be actioned over the coming weeks. 

 

Loss of Connectivity

There is currently a loss of connectivity for some of our customers attempting to access sites from the N3 network. We are investigating the cause of this and will provide updates here as soon as we have any new information. 

Update 1600hrs: We understand that BT had misdirected traffic bound for part of our IP address range, causing some of our servers to become inaccessible. This has now been resolved. 

 

N3 Outage

A cable fault on the N3 firewall cause a loss of connectivity for approximately 90 minutes between 0900hrs and 1030 hrs.

 

Partial N3 Outage

The ARP entries on the NHS facing firewalls were not present and required the static routes to the removed then added to the system. Service disruption lasted for 90 minutes. 

 

Partial N3 outage

There was partial outage on our N3 connection following BT's switch over of our line from NHSnet. This was due to  BT not migrating all our IP addresses, hence some sites were accessible from N3 while others were not. Internet connectivity was not affected. Total outage was 18 hours, most of which was out of office hours.

24 hour NHSnet connectivity outage

NHSnet connection has been restored.
Our apologies for the time it took to resolve the issue.
BT have advised that they had removed our hosting environment from their routing tables. No explanation for how they permitted this to occur has yet been forthcoming.
 

2005

Unplanned server downtime: None

Other issues: NHSnet bandwidth capacity issues emerged at the end of 2005. 

Resolution: Additional bandwidth ordered. Subject to BT turnaround times, this will be available in March 2006


 Note: There are reports of sometimes severe bottlenecks on the main N3: internet gateway due to high demand. This is not part of the ioko/Carelink infrastructure but part of the main system implemented by BT for CfH. This is resulting in reduced internet service for many NHS organisations. This is outside our control, is not something we can influence and only affects organisations accessing the internet by way of the NHSnet.
